Marriage

marriage license or certificate. |
Divorce

remove
ex-spouse from current medical and dental coverage; §
cancel life
insurance coverage on ex-spouse; §
change life/retirement beneficiaries (can be done at any

A copy of the first and last page of divorce decree; OR ·
A copy of the
letter from the attorney or judge. |
Birth/adoption of dependent
|
What
changes can you make? §
add newborn to
your current medical and dental coverage; §
increase the amount
allotted to Medical FSA; §
enroll in
Dependent Care FSA subject to IRS guidelines; §
start coverage
for newborn under your current life insurance; §
change life/retirement beneficiaries (can be done at any

Confirmation of Birth letter from the hospital; OR · A copy of the birth certificate; OR · A copy of the adoption papers. |
Loss of dependent’s eligibility (dependent
turns 26, graduates, gets married, or turns 19 and is not a full-time student)

from spouse’s employer/ insurance company stating termination of coverage,
effective date, and who was covered. |
Enrollment in other coverage

spouse’s employer/ insurance company showing new coverage effective date. ·
OR a copy
of your new insurance card (must show effective date of coverage). |
Important: Letters
from employers, judges, attorneys, hospitals, or doctors must be on official
letterhead or stationary of the person or company verifying the change.
