Marriage

 

What changes can you make?

§   add new spouse to your current medical and dental coverage;

§   increase the amount allotted to Medical FSA;

§   start coverage for spouse under your current life insurance;

§   change life/retirement beneficiaries (can be done at any time).

 

What documentation is required?

·   A copy of your marriage license or certificate.

 

 

 

Divorce

 

What changes should you make?

§   remove ex-spouse from current medical and dental coverage;

§   cancel life insurance coverage on ex-spouse;

§   change life/retirement beneficiaries (can be done at any time).

 

What documentation is required?

·                A copy of the first and last page of divorce decree; OR

·                A copy of the letter from the attorney or judge.

 

 

Birth/adoption of dependent

 

What changes can you make?

§   add newborn to your current medical and dental coverage;

§   increase the amount allotted to Medical FSA;

§   enroll in Dependent Care FSA subject to IRS guidelines;

§   start coverage for newborn under your current life insurance;

§   change life/retirement beneficiaries (can be done at any time).

 

What documentation is required?

·  A copy of Confirmation of Birth letter from the hospital; OR

·  A copy of the birth certificate; OR

·  A copy of the adoption papers.

 

 

Loss of dependent’s eligibility (dependent turns 26, graduates, gets married, or turns 19 and is not a full-time student)

 

What changes should you make?

§   remove dependent from current medical and dental coverage.

§   cancel dependent’s life insurance coverage.

What documentation is required?

Not required.

 

Loss of other coverage by spouse

 

What changes can you make?

§   add spouse/covered eligible dependents to your current medical and dental coverage;

§   enroll in medical and dental coverage on self if previously covered by spouse’s policy.

What documentation is required?

·         A copy of a letter from spouse’s employer/ insurance company stating termination of coverage, effective date, and who was covered.

 

 

Enrollment in other coverage

 

What changes can you make?

§   drop your current medical and/or dental coverage.

 

What documentation is required?

·         A letter from spouse’s employer/ insurance company showing new coverage effective date.

·         OR a copy of your new insurance card (must show effective date of coverage).

 

 

Important: Letters from employers, judges, attorneys, hospitals, or doctors must be on official letterhead or stationary of the person or company verifying the change.
