A documented narrative account of performance may be substituted for the evaluation form as long as this narrative is comprehensive in nature. The manager should apply the same ratings used on the annual evaluation form in a narrative.  The narrative should include specific references to accomplishments during the review period, document performance challenges, and recommend opportunities for improvement.  Finally, it should address goals or work standards and a professional development plan for the coming year.
