Important 2012 FSA Activation Required

  • The U.S. Bank Health Savings Solutions website has changed to www.mycdh.usbank.com and requires account activation.
  • Log on to "Create you new user name" and "password" under New User?
  • A "New User" box will appear. Follow the onscreen instructions, filling in your first name, last name and ZIP code, and your Employee ID. Select the "Next" button.
  • The system will generate a username for you. You will need to create a 7-10 character password that includes at least one number. Please make a note of your username and password and keep it safe. Select Next to continue.
  • Select and answer five security questions and select Submit. For security reasons, you may be asked one of these questions when completing certain account functions. If prompted, review and accept any required disclosures.
  • Congratulations! Your account is activated and you can now designate or update beneficiaries, order additional cards for dependents, and set up your bank information for convenient reimbursement options.

Georgia Tech offers two great ways for you to set aside pre-tax dollars from your paycheck to reimburse you for qualified dependent day care and out-of-pocket health care (medical, pharmacy, dental, and vision) expenses through participation in a Flexible Spending Account (FSA). U.S. Bank is the FSA administrator for Georgia Tech.

Employees - Regular full-time/part-time faculty or staff who work at least 30 hours per week.

Healthcare Spending Account

A Healthcare FSA is an account that provides you, your spouse, and eligible dependents with pre-tax reimbursement for qualified healthcare (medical, pharmacy, dental, and vision) expenses that are not covered by insurance. You can set up automatic reimbursement for orthodontia treatment by completing the Automatic Orthodontia Request form.

Healthcare deductibles, co-insurance, co-pays, prescriptions, glasses, orthodontia, certain over-the-counter medicines and supplies are eligible if incurred while you are participating in the plan. For a comprehensive list of qualified healthcare expenses, review the FSA eligible expense list. U.S. Bank will provide you with a debit card to pay for your qualified expenses.

If you have experienced a qualifying life event such as marriage, divorce, birth of a child, or your spouse's employment status has changed; you have 31 days to notify the Benefits Team at 404-894-1417 and provide proof of your qualifying life event. To make changes to your FSA as a result of one of these qualifying life events, complete the FSA Change Form and fax to the Benefits Team at 404-894-6978.

Dependent Day Care Spending Account

A Dependent Day Care Spending Account is an account that provides pre-tax reimbursement for your eligible dependents' childcare expenses. Under certain circumstances, the account may be used to help pay for the care of elderly dependents or a disabled spouse or dependent. U.S. Bank will provide you with a debit card to pay for your qualified expenses or you can set up Automatic Dependent Day Care Reimbursement with U.S. Bank.

Eligible dependent day care expenses may include services inside or outside your home by anyone other than your spouse, a relative, or a person you list as a dependent for income tax purpose or one of your children under the age of 19. Services may be provided at a child or adult care center, nursery, preschool, after school, or summer day camp. This care must be for:

  • a dependent child under the age of 13 years old, who lives with you for more than half the year and is supported by you;
  • a dependent child older than 13 or other relative who is incapable of self care and requires care while you are at work;
  • a non-relative you support who is incapable of self-support.

Annual Contribution Maximums

  • You may elect to contribute up to $5,000 annually toward the healthcare spending account.*
  • You may elect to contribute up to $5,000 annually toward the dependent day care spending account.* 

* If you are married and file jointly, your combined contributions may not exceed $5,000.00.

* If you are married and file separately, your individual contributions may not exceed $2,500.00.

Effective Date

Enrollment in the Flexible Spending Account is effective the first day of the month after your enrollment is submitted.

Changes to Your Election

Due to restrictions imposed by the Internal Revenue Service (IRS) code, after you make enrollment decisions during your initial eligibility or open enrollment, you cannot make changes during the remainder of the plan year (Jan. 1 to Dec. 31) unless a qualifying change in family status occurs. Please see the Family Status Change section for more information. At the time of your qualifying family change, you may not reduce your annual election to lower than what you have contributed year-to-date at the time of the change to your election. To make changes to your FSA as a result of one of these qualifying life events, complete the FSA Change Form and fax to the Benefits Team at 404-894-6978.

Processing Claims

You may use the Mastercard debit card provided to you by U.S. Bank or you can submit paper claims. For any claims paid using the debit card that are not co-pays under the medical, dental, or vision plans, you must substantiate proof of their eligibility within 60 days to U.S. Bank. You are required to file all paper claims for reimbursement within 90 days from the end of the plan year or within 90 days from the date you are no longer eligible to participate in the plan. You can file your claims by submitting the FSA Reimbursement Claim Form or by going online and providing your claims detail on the U.S Bank web site. You can also set up automatic Dependent Day Care Reimbursement for reoccurring dependent day care expenses.
